Information on CERT Documentation Requests-All Providers

Effective October 11, 2005 the Comprehensive Error Rate Testing Program (CERT) will transition documentation handling of new medical record requests and receipts from the CERT Contractor in Richmond, Virginia to the CERT Contractor in Annapolis Junction, Maryland. Please note the following instructions:
  • Please refer to the Request for Medical Records letter to ensure prompt and accurate medical record submission
  • The fax number (preferred method) for submitting medical records to the Maryland CERT contractor is (240) 568-6222. The CERT contractor will send a letter to the provider confirming receipt of the faxed medical records.
  • The correct address for submitting medical records via mail. Some providers may receive requests from two different contractors during this transition period. Mailed medical records are at the providers expense.
  • Submit All Medical Records for the claim date of service requested. Submission of partial medical records will result in a denial for insufficient information.
  • Include the bar code cover sheet with the medical records
  • Submit the medical records timely to avoid claim denial.
  • Failure to respond to requests for medical records by the CERT Contractor will result in recoupment or denial of payment to the provider.
Compliance with the CERT program is mandated by the Centers for Medicare and Medicaid Services and does not violate HIPAA.
