Procedure
  • 1. Complete the MPTA Institute Filippis Scholarship Grant Form ( PDF: 343kb) and submit to the Michigan Physical Therapy Association Institute for Education and Research, Inc.
  • 2. Submit your MPTA Financial Aid Form (FAF) to the financial aid officer of your collegeor university to complete.
  • 3. Financial Aid office is to send completed application to:

      Ann Arbor, Michigan 48104-4294
      ATTENTION: Education Awards Committee
  • 4. Have official transcripts of all college course work sent to above address by college office.
  • 5. Have recommendations sent to the above address by the person completing the recommendation form.
  • 6. An interview may be required and would be set up by the Institute.
  • 7. This scholarship is for students in the final year of their program.
  • 8. Deadline to receive all materials: July 27, 2007.
